Existing Clubs and Leagues
We are still affiliating Clubs and receipts will be sent out with the handbook. Clubs may request their affiliation number from the Governance Office.
Affiliation Guides are available to download as is the summary for Public Liability Insurance Cover for 2008/09.
League Affiliations need to be in the office before 1 August 2008, any League who does not have the League Affiliation forms please request them from the Governance Office.
Category of Membership and Cup Form
This form has the affiliation and the County Cup fees, again this will be included in the affiliation pack for existing Clubs to sign and return but this will need printing off, completing and sending with the affiliation for new clubs.
New Clubs
Blank Affiliation forms and a guide of how to complete the forms are available to download.
Please complete the following documents and send to CCFA along with a cheque or postal order:
1. Completed & Signed Affiliation Form
2. Completed & Signed Category of Membership Form
3. List of Players for the current season
List of Players
Information from all of your players such as name, data of birth, and address are required.
This information is required to affiliate your Club, existing Clubs will have pre-populated details which you will be able to amend/ add where necessary.
New Clubs are now able to download this part of the affiliation form.
The request for this information does not contravene the Data Protection Act. Cheshire FA's registration within this Act allows the use of such data for administration purposes.
Insurance
A Summary of Public Liability Cover, which is compulsory to affiliate to Cheshire CFA, as is Personal Accident Insurance has been provided to download here for your information.
