Affiliation 10/11
For Existing Clubs the Affiliation forms for existing Clubs were sent out on Wednesday 7th April 2010.
Please send in the completed form by 1st July 2010, please note if the forms or cheque are incorrect then it will be sent back to you.
For New Clubs wishing to affiliate to Cheshire FA please download the following documents (on right of the screen):
An Affiliation Guide for Clubs 10/11 is available to download, please ensure that you read this carefully before completing and returning the affiliation forms.
Please complete the following documents and send to CCFA along with a cheque or postal order:
1. Completed & Signed Affiliation Form including List of Players for the current season
2. Completed & Signed Category of Membership Form
3. Completed & Signed County Cup Form
4. Club Insurance Declaration Form
Insurance
Public Liability Cover, which is compulsory to affiliate to Cheshire CFA, as is Personal Accident Insurance.
