Affiliation 09/10
For Existing Clubs the Affiliation forms for existing Clubs were sent out on Friday 3 April 2009.
Please send in the completed form by 1 July 2009, please note if the forms or cheque are incorrect then it will be sent back to you.
For New Clubs wishing to affiliate to Cheshire FA please download the following documents (on right of the screen):
An Affiliation Guide for Clubs 09/10 is available to download, please ensure that you read this carefully before completing and returning the affiliation forms.
Please complete the following documents and send to CCFA along with a cheque or postal order:
1. Completed & Signed Affiliation Form including List of Players for the current season
2. Completed & Signed Category of Membership Form
3. Completed & Signed County Cup Form
4. Club Insurance Declaration Form
List of Players
Information from all of your players such as name, data of birth, and address are required.
This information is required to affiliate your Club, existing Clubs will have pre-populated details which you will be able to amend/ add where necessary.
New Clubs are now able to download this part of the affiliation form.
The request for this information does not contravene the Data Protection Act. Cheshire FA's registration within this Act allows the use of such data for administration purposes.
Insurance
Public Liability Cover, which is compulsory to affiliate to Cheshire CFA, as is Personal Accident Insurance.
